When you have a dedicated customer signup? What do you give to them to sign etc and such forms before they get their services? Also for colo.

I currently haev the following ready:

*Credit Card Authorisation Form
*Service Agreement

Anything else you guys do on startup of a new customer? That requires customers input also.

I ask for more information from a client for dedicated servers than I do for shared hosting. It is a bigger commitment, so I ask for bigger references. I verify phone numbers, contact emails etc before I release their server. Another thing, I charge a setup on servers, since it seems to weed out any hackers a little better.

The client needs to agree to TOS and AUP. As well as SLA of the datacenter (plus TOS and AUP of the datacenter if it is different that yours)

Oh, and I have in the past (when I was suspicous for no reason other than they wanted 4 servers) asked that the server setup fees be paid by money order. They didn't even blink, and the server has had no problems.

One of the things we now do is require that we are allowed to put on mail monitoring software. This has chased away a few people that I was uncomfortible with from the start. Funny how I've never had a problem with a dedicated client who had the software on their box.

Basically i have the following setup for when a customer orders such a product... They must agree to TOS/AUP/SLA. These three policies along with the contractual agreement are written up as one document, ie. Section A is the service agreement, Section B the AUP, Section C TOS and so on. I require that they sign this and mail it back to me (as faxing 20 pages is a bit much heh).

I was just wondering would it be possible for me to upload the service agreement/tos/aup/sla and then just have a single sheet document which shows the cnotract spec (ie. what they ordered, howmuch for and the min. term) and then on that one document have them tick boxes to say they have accepted the tos/aup/sla and the service agreement. My main reason for this is 1 page is so much more 'easy' for the client to process than going through a bunch of other stuff. Yet on the form they have agreed that they have read and accept the other terms too.

RackShack customers...do you have any forms to fill and fax/mail in? Or is it all done electronically? My main concern of doing it all electronically is fraud etc.

Adam - for month to month basis - a digitally signed (tick box) would be adequest - make it so they can not get past the signing order without ticking the tick box. Therefore they have signed it digitly - if they complain about something and try take you to court - you post your web site to the judge - give me serveral hours experienc eon a cpmputer ;) and then say look he has to tick this - and make it hack proof etc etc.

For contract - ie 12 month - send them a fax which tey have to sign. On this fax you could have something that has an address to the TOS/AUP and SLA. Also before they sign the contract make them digitially sign it as well online - 2 proofs her e:)

2 proofs would not be needed. YEah month to month i guess digital signature is adequate. But im thinking of imposing a mimum terms whether it be 3 or 6 months - im kinda against 12 months contracts. Main reason for 3 month minimum terms is so that if they leave, im not stuck with any outstanding charges to pay of (ie when i buy a server for them).

When they go to the signup form then need to agree TOS/AUP/SLA then fill out all the information. Once then fill out they got automatic email that got TOS/AUP/SLA and tell them that it take 24 hrs setup.

As far as things to sign, we don't make customers sign anything unless they are asking for SSH/Telnet and then they must fax us a copy of their signature agreeing to our SSH/Telnet terms. However, we make everyone agree to our terms on sign up.

When clients signup with us, they are required to mark a box saying they read and agree to our TOS. We lease our servers on a month to month basis so we feel this is adequate. Clients requiring SSH access are also required to scan/e-mail or fax a photo copy of their photo id.
